Who should I contact?

I haven’t heard from the hospital and I’m worried that they may not have received my referral.
I cannot attend the outpatient appointment I have been offered.
I cannot attend the endoscopy appointment I have been offered.
I cannot attend the scan appointment I have been offered.
I cannot attend the date I have been offered to have chemotherapy.
I cannot attend the date I have been offered to have surgery.
I’m upset and worried about my diagnosis or treatment.
I’m worried about how my diagnosis is going to affect my life, including my work, money, hobbies, relationships etc.
I have specific questions about my diagnosis or treatment that I’d like to ask.
I have a compliment, complaint or feedback to make about the care or treatment I have received.
I am a family member/carer of someone affected by cancer. Is there any support available for me?
I was supposed to have a follow up appointment, but I haven’t been contacted by the hospital.
