Question Number 1: I am submitting a Freedom of Information request for the: Procedures that are used by the hospital for the assessment of people being discharged from hospital. I understand that patients will need to be assessed individually and pathways for discharge will be influenced by this. I am not asking for personal information I am requesting the generic policy, procedures and guidance that are used to ensure that a patient is discharged safely from hospital and with the appropriate level of safe care. | |
Answer To Question 1: Please find the BSW Leaving Hospital policy (currently under review at SFT and some of the appendices we don’t use, the information has been superseded by more recent information regarding discharge issued by BSW and available on this link Discharge Communications - resources for patients - BSW Together ) and the following link that provides overarching information regarding what acute hospitals consider as reasons to remain in hospital , or to consider discharge plans. Hospital discharge and community support guidance - GOV.UK (www.gov.uk) Although produced by BSW, we use the principles of the policy in relation to Hampshire, Dorset and patients coming from further afield.
