Question Number 1:
I have the following Freedom of Information questions to ask about the Cardiac Rehabilitation department at Salisbury District Hospital:

Once the patient has completed the 8 week Phase III Cardiac Rehabilitation programme, what is the policy of offering or recommending independent outside clubs, groups and professional agencies for their continuing follow on care as this is is not provided for free by the ¾¨Ó㴫ý.
 
Answer To Question 1:
Once the patients finish with their ¾¨Ó㴫ý treatment patients are not routinely recommend any specific clubs/groups or professional agencies. If a patient has ongoing issues they remain within the Cardiac Rehab phase 3 program until their issue has resolved . The patient may of course have ¾¨Ó㴫ý Cardiology follow up
 
Question Number 2:
On the Phase III Cardiac rehabilitation course it was recommended by the nurses and team that there were other agencies and groups available to provide the follow on services (Phase IV) as these were not provided by the ¾¨Ó㴫ý, is this still the case and if not why not?
 
Answer To Question 2:
The patients are informed of the providers of Phase Cardiac Rehab and there are up to date information on their location and class times. Leaflets or written information regarding the providers are not given to patients.
 
Question Number 3:
If this practice is still carried out. How does an outside agency get recommended to the patients by the nurses and team on the Phase III course?
 
Answer To Question 3:
Once a patient competes the Phase 3 programme if they wish to be referred on to Phase 4 CR a member of the team will ask which provider they wish to be referred and then complete a BACPR transfer form and email to the provider/ or for any Active health referrals a direct referral form is completed in the active health online hub.

On the occasion a patient wishes to go to a provider outside of area a search is undertaken for their area. Once assured that they are Level 4 Cardiac qualified the patient is provided with their BACPR transfer referral form so that they can hand this to the provider they have chosen to go to/ or email the transfer form directly if this is an option.

A record of where the patient has chosen to go is entered into the Phase 4 referral book that is kept in the Cardiac Rehab Teams office. The provider then contacts the patient directly or in the case of the hospital leisure centre the patient calls in to arrange an appointment
 
Question Number 4:
Are the nurses in the Cardiac department allowed to provide Phase IV Cardiac rehabilitation courses privately outside of the ¾¨Ó㴫ý?
 
Answer To Question 4:
Yes and the Trust is informed in the event this happens.
 
Question Number 5:
Can you supply a list of organisations, independent outside clubs, groups and professional agencies who undertake continued follow-on care and are recommended by the Trust?
 
Answer To Question 5:
No. The trust does not recommend anyone specifically.
