Question Number 1: Would you be able to confirm if: - paper notes/drug charts are used anywhere in the trust, alongside electronic paper notes/drug charts? - if so, are there any plans to migrate away from paper to electronic? if not, how often is the decision reviewed? - if so, does the trust keep track of serious incidents etc related to paper specifically eg misreading of handwritten comments/doses? - How many incidents happened within the last year (or period the trust tracks)? - if not, how long ago did the trust migrate away from paper notes? | |
Answer To Question 1: The Trust still has paper notes/drug charts in use in a range of areas. The Trust is currently procuring a new Shared EPR and as part of that process there will be a decision on the best approach to migrate from paper to electronic for all remaining services. The Trust's incident management system will have incidents recorded which might as part of it include issues related to paper but there is not dedicated reason in the product for paper records incidents so it is hard to accurately report this. | |
